If you wish for your child/student to attend and participate in the carnival, please complete the Medical and Consent forms (PDF, 542KB) and return all pages to your child's form teacher by Monday, 22 February 2021.
If parents would like to submit a Digital Permission Note (DOC, 9092KB), please DOWNLOAD (right-click, Save-As) the Microsoft Word document below to your own computer. Fill out the form, then SAVE it as your Student's Name and Year Level. Please then send an email to Ms Deb Casey: dcase56@eq.edu.au with the note attached.
